Resumen
Os factores lipídicos tradicionais de risco permitem prever apenas alguns dos eventos cardiovasculares. Tem sido nosso objectivo o estudo e a procura de novos marcadores biológicos de risco, nomeadamente a lesão eritrocitária e a activação leucocitária. Estudámos estes potenciais marcadores de risco em doentes em risco de eventos cardiovasculares, em casos de enfarte de miocárdio e de acidente cérebro vascular isquémico e em adolescentes praticantes de exercício físico moderado e de alta competição. Foram também realizados estudos in vitro a fim de clarificar o valor dos marcadores propostos. Observámos nos doentes um aumento de leucócitos, que parecem activados, como evidenciado pelo aumento dos seus produtos de activação, elastase e lactoferrina. Esta activação parece impôr lesões e um envelhecimento prematuro dos eritrócitos, como sugere o aumento de hemoglobina ligada à membrana e um distinto perfil de banda 3 observado. Nos estudos in vitro verificou-se que as modificações impostas no perfil da banda 3 por neutrófilos activados e por elastase em concentrações crescentes se correlacionavam e eram semelhantes às apresentadas pelos eritrócitos mais velhos. Os nossos resultados sugerem que a concentração plasmática de elastase e lactoferrina, em conjunto com os valores de hemoglobina ligada à membrana e o perfil da banda 3, devem ser estudados como potenciais marcadores de risco de eventos cardiovasculares.

Abstract
The traditional lipid risk factors can only predict some of the cardiovascular events. Our recent work has focused on new potential biological markers of risk, namely erythrocyte damage and leukocyte activation. We have studied these potential markers in patients at risk for cardiovascular events, in myocardial infarction and ischemic stroke cases and in adolescents practising moderate and high competition physical exercise. Some in vitro studies were also performed to strengthen the value of the proposed markers. We found a rise in leukocytes which seem to be activated, as shown by the increase in their activation products, elastase and lactoferrin. This activation seems to impose erythrocyte damage and premature aging, as suggested by the increase in membrane bound hemoglobin and by a different band 3 profile. The in vitro studies showed that the modifications imposed to band 3 profile by increasing concentrations of activated neutrophils and elastase were correlated, and were similar to those presented by older erythrocytes. Our data suggest that plasma levels of elastase and lactoferrin, together with levels of erythrocyte membrane bound hemoglobin and band 3 profile, warrants further studies as potential new markers of risk for cardiovascular events.
