ACTUALIZAÇAO ECOCARDIOGRAFICA EM PACIENTES CON DISFUNÇAO DIASTOLICA

Resumen
Actualmente é incontestável a (es irrefutable la) participação da disfunção diastólica como componente da insuficiência cardíaca e fundamental o seu (su) reconhecimento para um diagnóstico e prognóstico adequado. Por outro lado, o compromisso da função diastólica pode interferir na função (en la función) sistólica e vice-versa, de tal forma que é necessário que a função ventricular seja avaliada (sea evaluada) de forma integrada, considerando-se todas as (las) fases do ciclo cardíaco. Este artigo revê a (revisa el) análise não invasiva da função diastólica por ecocardiografia abordando as várias técnicas disponíveis: análise do fluxo (del flujo) mitral, do fluxo de veias pulmonares, da velocidade de propagação do fluxo pelo (por el) modo M, do Doppler tecidual (tisular) e do (y del) volume auricular esquerdo. Particularmente a análise do fluxo transmitral e o Doppler tecidular do anel mitral, com a obtenção da razão (de la razón) E/A e E/e', estão amplamente difundidas para determinação das pressões de enchimento do (llenado del) ventrículo esquerdo. Contudo a (Sin embargo) existência de limitações de ordem clínica e técnica levaram ao desenvolvimento (llevaron al desarrollo) de novas tecnologias e à (y la) investigação crescente nas modalidades derivadas do speckle tracking (strain, strain rate, torção), que permitem a avaliação da (la evaluación de la) função ventricular com maior independência de factores extracardíacos. Espera-se que a (Se espera que la) validação destas (de estas) novas técnicas conduza à melhoria do (lleven a la mejoría del) conhecimento na avaliação da função diastólica.

Abstract
Participation of diastolic dysfunction as a component of heart failure is nowadays unquestionable and its recognition is vital for a proper diagnosis and prognosis. Moreover, the impairment of diastolic function can interfere with systolic function and vice versa, requiring the evaluation of ventricular function in an integrated manner, considering all phases of cardiac cycle. This article reviews the analysis of diastolic function by non-invasive echocardiography, approaching the various techniques available: analysis of mitral inflow, pulmonary venous flow, color M-mode flow propagation velocity, tissue Doppler and left atrial volume. Specially the analysis of mitral inflow and tissue Doppler, with ratio E/A and E/e' are widely held to determine the filling pressures of left ventricle. However, technical and clinical limitations led to the development of new technologies and growing research in modalities derived from speckle tracking echocardiography (strain, strain rate, twist), which allow the assessment of ventricular function with greater independence of extra cardiac factors. It is expected that the validation of these new techniques lead to the improvement of diastolic evaluation knowledge.
