UTILIDAD DE LA ULTRASONOGRAFIA ENDOSCOPICA EN EL DIAGNOSTICO DE LOS LINFOMAS

La ecoendoscopia es útil para la estadificación de los linfomas gastrointestinales, así como para el diagnóstico de linfomas localizados en órganos o de adenopatías de difícil acceso cuando se acompaña del análisis citológico del material obtenido mediante punción aspiración con aguja fina o biopsia.

Resumen
La ecoendoscopía digestiva es una herramienta de gran utilidad para el diagnóstico y estadificación de las neoplasias digestivas, así como para aquellas que comprometen estructuras cercanas al tracto digestivo. Los linfomas son neoplasias que albergan una elevada dificultad diagnóstica, y ello se debe a la íntima relación entre el subtipo histológico y la respuesta al tratamiento quimioterapéutico. De esta forma, un diagnóstico preciso es fundamental en el manejo de la enfermedad. Mientras que la mayoría de los linfomas gastrointestinales pueden ser diagnosticados y subclasificados a partir de biopsias endoscópicas, los que afectan a órganos o adenopatías de difícil acceso requieren con frecuencia maniobras invasivas para la obtención de una muestra histológica adecuada. La ecoendoscopía ha demostrado su utilidad en la estadificación de linfomas gastrointestinales y, además, puede ser útil en el diagnóstico de linfomas localizados en órganos o adenopatías de difícil acceso cuando se acompaña del análisis citológico obtenido mediante punción aspiración con aguja fina o biopsia. En este artículo se revisa el papel de la ecoendoscopía digestiva en linfomas gastrointestinales, pancreáticos, esplénicos o nodales.

Abstract
Endoscopic ultrasound is a useful tool in the diagnosis and staging of gastrointestinal neoplasms, as well as in those that involve structures in the vicinity of the digestive tract. Lymphomas are tumors that reveal a high diagnostic difficulty because of the close relationship between the histological subtype and the response to chemotherapy. Therefore, an accurate diagnosis is crucial in the management of the disease. Whereas most gastrointestinal lymphomas are diagnosed and sub-classified using endoscopic biopsies, those involving deep-seated organs or lymph nodes often require invasive procedures to obtain an adequate histological specimen. Endoscopic ultrasound has proven to be accurate in the staging of gastrointestinal lymphomas and may be useful in diagnosing deep-seated lymphoma when fine needle aspiration cytology or biopsy is performed. This manuscript reviews the role of endoscopic ultrasound in gastrointestinal, pancreatic, splenic or nodal lymphoma.
